Why are we excited about Gravitate Health from a healthcare system and patient empowerment perspective?
This session will bring together partners of the Gravitate Health consortium and a wide audience for an open discussion on the success factors that are important to create, implement, and sustain digital tools and services that are accessible to entire populations while allowing for a sophisticated level of personalisation at the same time.
Gravitate Health project will be presented through the concept of different G-Lenses, as personalized, patient/citizen-centric solution. This concept incorporates ecosystems’ knowledge, sustainability principles and is grounded on Interoperability approach and easy integration with Health Tech solutions.
Collaboration among stakeholders within the healthcare sector is the crucial factor towards more transparent networks of ecosystems that put the patient at the center of care.
